Client Acknowledgement & Indemnity
A plain-language acknowledgement to support informed consent and safe tattooing.
By proceeding with a tattoo appointment, the client confirms that the information they provide is accurate and complete and that they understand tattooing involves puncturing the skin and carries inherent risks including irritation, allergic reaction, infection, scarring, pigment change and variable healing.
Client declarations
- I am legally able to consent to the tattoo, or the required lawful consent has been obtained where applicable.
- I will disclose relevant medical conditions, allergies, medication and skin concerns that may affect the procedure or healing.
- I am not attending the appointment intoxicated or impaired.
- I understand that final appearance, colour retention and healing vary between people and body areas.
- I have had an opportunity to review the design, spelling, placement and orientation before tattooing begins.
- I accept responsibility for following the aftercare instructions supplied by the artist.
- I understand that touch-up requirements, rescheduling, cancellation and deposit arrangements are subject to the booking terms agreed with the artist.
Onsite appointments
For mobile or onsite tattooing, the client or host must provide an area that can be made clean, private, adequately lit and free from pets, smoking, food preparation and unnecessary foot traffic. The artist may refuse or stop work where hygiene, safety or professional conditions cannot be maintained.
Indemnity
To the extent permitted by applicable law, the client accepts the ordinary and disclosed risks of tattooing and agrees not to hold After Dark Tattoos or the artist responsible for consequences caused by undisclosed health information, failure to follow aftercare, interference with healing, exposure to unsuitable environments, or conduct outside the artist’s reasonable control. Nothing in this acknowledgement excludes rights or liability that cannot lawfully be excluded.
